Collecting Info with Intake Forms
How to attach a custom intake form to a registration in Admittee, the field types available, section collection frequency, the registrant checkout experience, and reviewing or exporting responses.
Intake forms let you collect extra information from registrants when they sign up — t-shirt sizes, dietary needs, emergency contacts, waivers, and more. You attach one custom form to a registration, choose which fields appear, and review or export every response from the registration's stats table.
Adding a form to a registration
Open the registration in the dashboard and go to the Form tab. You have two ways to start:
- Upload an existing form. Drop in a PNG, JPG, WEBP, or PDF of a form you already use. Admittee reads the file with AI, extracts the title, fields, and any options, and turns it into an editable form. You can adjust anything it picks up.
- Create new. Build the form from scratch, field by field.
Once a form exists, you build it out by adding sections and, inside each section, fields.
Field types
Each field maps to a specific input. The available types are:
- Name, Text input (single line), and Long text input (paragraph)
- Checkbox (select several options) and Multiple choice (pick one)
- Dropdown (pick one from a list)
- Phone, Email, and Date
- Consent — display agreement or waiver text with an "I agree" checkbox
- Signature — registrants sign with a pen/touch or by typing their name
- File upload — attach documents or images
- Additional information — display-only text or instructions, not a fillable field
For checkbox, multiple choice, and dropdown fields you enter the list of options. Any field can be marked required.
Collection frequency
Each section has a collection frequency that controls how often it's shown:
- Collect once — asked one time per order.
- Collect per registration — repeated for each spot in the order, so multi-participant sign-ups answer separately.
- Collect per [type] — limit a section to a specific registration type.
The registrant experience
At checkout, registrants see the form before completing their registration. Required fields must be filled in before the Continue button enables. Phone and email entries are validated, dates use a picker, and uploaded files appear in a list they can remove.
Reviewing and exporting responses
Responses land in the registration's stats table. Each row that submitted a form shows a Form icon — click it to open that registrant's full response.
You can also surface answers as columns: open the gear/columns drawer and turn on the custom-field columns (hidden by default). To pull everything out, use Export to download a CSV with one column per form field.
